Pet Cost Planning Tools

Quick answer: keep a dated record of the foods involved, package details, what you observed, and when you contacted your veterinary team. This worksheet helps organize a food-change conversation; it does not recommend a food, set a transition schedule, or diagnose a reaction.

Reviewed November 3, 2026. Method: the fields follow FDA’s pet-food complaint information checklist, including product identification, signs, timing, and veterinary records. Use the form on this device or print it. Entries are not sent or saved by this page, so save or print a copy if you need to keep the record.

Pet and discussion details

Record both products as labeled

Copy details from the package when available. FDA notes that the exact product name, lot number, and best-by date can help identify a product. Keeping the original packaging or a clear photo may help preserve those details.

Food previously offered

New food discussed or offered

Dates and observations

Record what happened without trying to interpret the cause. Include the date and approximate time, what and how much the pet actually ate (if known), appetite, stool or vomiting changes, energy or other signs, and any other food or exposure you know about. Leave unknown details blank.

Veterinary follow-up

If your pet appears ill after eating a food, contact your veterinarian. FDA advises stopping the product and calling a veterinarian when a pet has a health problem related to a food or treat. This worksheet does not determine whether a food caused a problem or what care is needed.

Sources and limits

This U.S.-oriented worksheet is for record keeping only. It does not prescribe a food, feeding amount, or transition method, and it cannot establish that a food caused a symptom. For label and budgeting context, see the pet food budget guide, food adequacy label guide, and FDA recall lookup guide. The portion worksheet and dog food cost calculator use amounts already chosen by the visitor; they do not give transition or diet advice.
