Organize a pet prescription refill and written quotes
Record prescription-label refill dates and organize written pharmacy quotes for the same veterinarian-prescribed animal drug.
Record the exact veterinarian-prescribed animal drug and the refill dates shown on the prescription label or confirmed by the veterinarian or pharmacy. Then organize up to three current written pharmacy quotes for that same prescribed product. This page does not recommend a pharmacy or compare treatment options.

Enter dates exactly as stated by your prescription label, veterinarian, or pharmacy. This worksheet does not infer when a refill is due, decide whether refills remain authorized, or store reminder alerts.
Written pharmacy quotes
Use quotes for the same exact prescribed product, form, and strength. Enter the quoted package quantity and its unit (for example, tablets or mL), the product price, confirmed delivery charge, and quote date. Do not enter doses or change the prescription to make offers appear comparable.

Method: quote total = quoted product price + confirmed delivery charge. Arithmetic per stated package unit = quote total ÷ quoted package quantity. This omits any taxes or fees you did not enter. Per-unit figures are arithmetic only and are not comparable when product, formulation, strength, package unit, or quote terms differ.
Check the product and prescription with official and professional sources
FDA’s Animal Drugs @ FDA database includes most FDA-approved and conditionally approved animal drugs; FDA notes that indexed drugs for certain minor species are listed separately. A database search can help locate product records, but it does not confirm that a quote is for the prescribed product or that a prescription can be refilled. For a mismatch or refill question, contact the prescribing veterinarian or pharmacy. FDA says an online pharmacy must have a valid prescription from a licensed veterinarian to sell a prescription veterinary medicine.

The worksheet adds the entered product price and confirmed delivery charge. It does not include taxes or fees unless you enter them in the delivery field; verify the final total and quote validity directly.
Can I compare quotes for different package sizes?
The worksheet shows arithmetic per entered package unit. Compare that figure only when the exact product, formulation, strength, units, and quote terms match; it does not establish equivalence or recommend a product.
Does the refill date tell me when I can reorder?
No. Record the date shown by the prescription label or confirmed by the veterinarian or pharmacy. Contact them to confirm refill authorization and timing. The tool does not calculate or remind you of a due date.
